Fit-To-Fly certificate
Do you need a Fit-To-Fly certificate for your flight? You can apply online through Keuringsartspraktijk. Once we receive your application a doctor will contact you by telephone to discuss your health and ask any additional questions. If the certificate can be issued, you will receive it digitally in English after the payment is completed. Continu reading below to find out how the procedure works and what our fees are.
How do you apply for a Fit-To-Fly certificate?
Step 1
Complete the form below with your personal details and a description of your symptoms. You can also attach photographs if relevant.
Step 2
One of our doctors will contact you by telephone to assess your situation. Additional information may sometimes be needed.
Step 3
If your application is approved, you will receive a payment link. After the payment is done, we will send you your medical certificate.
Fees
A standard application costs €100. This includes the telephone consultation and the certificate. For standard applications you will receive your Fit-To-Fly certificate within 5 working days. An urgent application costs €150. Do you need your certificate urgently? Please state this in your application, along with your flight’s departure date and time.

Check your airline’s requirements before applying. Ask which certificate you need and how recently it must have been issued. Have you received a specific medical form that needs to be completed? Please upload it with your application so the doctor can assess whether they can complete it.
